You can get a passport form using any of the following methods below;
 
1.
Collect it from a Post Office branch that offers the Check & Send service. Click here to find the nearest Post Office to you.
2.
Complete the application form online, the Passport Office will print out the completed form and send it to you to sign and return with the supporting documents
3.
Request an application form online and the Passport Office will send it to you in the post - allow up to five working days for delivery
4.
Call the Passport Adviceline 0300 222 0000 and ask the Passport Office to send it to you in the post - allow up to five working days for delivery
Filling in the Passport Form
There are now two ways to fill in a passport form.
Fill in the paper form
Completing form online, then printing

Filling in the paper form: You can obtain an original passport application form from the Post Office or sending an email via the Passport Office website or calling the Passport Office on 0300 222 0000 to get the form post to you.

Completing form online:  The online facility allows you to complete the entire application from your computer. This process can stop a lot of the errors that occur when filling in the paper form. Once you have finished completing it you print it or get the Passport Office to send it to you so you can put your signature and include any supporting documentation.
 
Whichever way you choose to fill in your form make sure you do it fully and completely and with clear, honest information.
Thoroughly read the supporting guidance booklet or online help before you start filling in your form and ensure you have all the documentation you need before you start.

 
When filling in the paper copy,
Complete the entire application with black ball point pen and
Use only capital letters throughout.
Write only in the white boxed areas and ensure that all characters fit in the boxes and do not go over the lines.
Leave all other parts of the form clear and make no markings anywhere else on the page.
